The Holding Group is a group of dedicated social workers\nwho are committed to providing you and your loved ones with high\nquality care. In 2024 The Holding Group became a provider for\nMedicare’s newest program\, titled ‘GUIDE’. GUIDE stands for guiding\nan improved dementia experience and is a longitudinal care coordination\nprogram that individuals with original Medicare now have access to free\nof charge. This program provides one on one support with a member of\na dedicated care team to help navigate our complex medical system\,\nprovide ongoing education and resource management support\, complete\nadvanced care planning\, and assist with understanding the Dementia\nprogression. Come learn more about the program\, what it includes\, and\nhow to get involved!
