The Holding Group
Do you or a loved-one struggle with Dementia? The Holding Group is
here to help! The Holding Group is a group of dedicated social workers
who are committed to providing you and your loved ones with high
quality care. In 2024 The Holding Group became a provider for
Medicare’s newest program, titled ‘GUIDE’. GUIDE stands for guiding
an improved dementia experience and is a longitudinal care coordination
program that individuals with original Medicare now have access to free
of charge. This program provides one on one support with a member of
a dedicated care team to help navigate our complex medical system,
provide ongoing education and resource management support, complete
advanced care planning, and assist with understanding the Dementia
progression. Come learn more about the program, what it includes, and
